Palliative Care Services Offered
Fountain Hospice and Palliative Care Services offers at home palliative care aimed at helping those with a life threatening illness to live comfortably in the familiar and reassuring surroundings of their own home.
Most terminally ill people want to spend as much time as possible in their own home surrounded by those they love. We understand that it takes courage and commitment from family members to provide continuous care at home.
Our services also aim to support family and friends, especially those in the role of carer, during this difficult time.
Our care team consists of registered nurses, counsellors, carers and qualified complementary therapists who work in consultation with your medical practitioner, paliiative care nurses and other health providers.
Our care is tailored individualy to each family and may include:
- liaison with your specialist palliative care nurses and medical practitioner as needed to provide symptom management
- carer education and support
- information on various choices for your care including complementary medicines and natural therapies
- routine scheduled visits
- counselling, spiritual support and guidance
- bereavement support for individuals and family members if requested
- carer respite by family support volunteers
Our services do not include direct nursing care at present. We are aiming towards including this facility and hope to receive funding in order to do so in the future.
The service is currently available in Sydney's Lower North Shore and Northern Beaches.
